In the disaster that was the April 20th Deepwater Horizon explosion, the ship’s blind shear ram, its last line of defense, failed.  However, the blind shear ram was not alone in its failures.  BP and Transocean failed to equip the Deepwater Horizon with two blind shear rams, and Minerals Management Service, the federal agency in charge of regulating offshore drilling, failed to require them to do so.  These three failures are ultimately are responsible for the tremendous extent of this catastrophe.
